    Camp 3: July 29 - August 2
    (If you are registering more than one child or one child for multiple weeks please complete registrations one at a time.)
    ​Video and Photographic Release:
    I understand that pictures or videos of my child participating in the soccer camp could be featured in Utah Elite Soccer advertisements, social media posts, website photos, etc. I understand that this is a requirement for participation.
    Emergency Treatment Program Waiver:
    ​I hereby authorize Utah Elite Soccer LLC staff to act on my behalf, in accordance with his or her best judgement, in case of an emergency involving me/or my child, and agree to assume full responsibility for all expenses, medical or otherwise, that may arise as a result of injury or accident sustained while participating in Utah Elite Soccer LLC activities. I understand that I, or my insurance company, will be billed for such emergency treatment.
    Liability Waiver and PAR-Q forms:
    I understand that I must fill out a liability waiver form and PAR-Q (health screening) form before my child may participate in Utah Elite Soccer LLC activities. These forms can be found below. I understand that it is strongly recommended that these forms are filled out prior to the event because in some cases a child may need to obtain a physicians release before participation.

    PAYMENT

    We have three options to take camp payment. Regardless of your selection you MUST complete the checkout process online to reserve your player's place in the camp.
    1) Credit/debit card payment on the website.
    2) Venmo payment: @UtahEliteSoccer. Please put player name, camp week, age group, and location as the transaction comment. **Use coupon code VENMO during checkout process.**
    3) Cash/check payment on FIRST day of camp. Payment will be required before your child may participate. **Use Coupon Code CASH during checkout process.**
